Complaints submitted anonymously will not be accepted. Written forms must contain your contact information and signature. You may keep your information confidential from the person against whom you are complaining, however, the forms must be filled in completely with your address, phone number and signature. Click on the button below for the form of your choice. To file a written complaint or request, click on one of the buttons below. Print the form, sign it and then send it in to us using the information at the top of the form. After signing the form, you may also scan it back into your computer and email it to: mhecker@spokanecity.org
